Percy was born in Derby, on the 3rd July, 1883 (according to his attestation form). He enlisted in Dauphin, Manitoba, 22nd January 1915, stating his occupation as a farmer, and his next of kin as his mother, Mrs. T. H. Boam, of Kedleston Road, Derby. He had previously served with C Squadron of the Derbyshire Yeomanry.
He was five feet seven inches in height, had a fair complexion, blue eyes and fair hair, and belonged to the Church of England. He had a scar on the right of his stomach.
